We Provide Compassionate Support for Aging and End-of-Life Transitions
Hannah Pasternak, LCSW

Hannah Pasternak, LCSW brings a grounded, compassionate presence to her work with individuals navigating aging, grief, loss, and life transitions. She supports clients who feel overwhelmed, emotionally stuck, or unsure how to regain a sense of steadiness during complex or painful chapters of life. Hannah’s approach is holistic and person-centered, integrating evidence-based modalities such as CBT, DBT, EMDR, and mindfulness to help clients process past experiences, regulate emotions, and move forward with greater clarity and self-trust.
With experience supporting clients through trauma, grief, anxiety, depression, and major life stressors, Hannah creates a space where clients can slow down, feel heard, and reconnect with what matters most. She works with adolescents and adults via TeleHealth throughout California, making therapy more accessible during times when in-person care may feel difficult or impractical.
What to Expect From Professional Care
During the first session, we thoroughly assess the individual’s physical health, emotional state, cognitive abilities, and social support network. Then, we develop a customized treatment plan. Throughout therapy, clients gain strategies to build resilience, manage stress, and cope with changes in their lives. We also work closely with families and caregivers.
We provide therapy and support for individuals of all ages, including those navigating the challenges of aging and end-of-life in Los Gatos, helping them embrace this journey with dignity and grace.
